All facilities within the state seeking license verification (hospitals,
nursing homes, or any other facilities that employ licensed Speech-Language
Pathologist or Audiologist) should email, fax or telephone their request to the
Board office. There will be no charge for this service.
A $25.00 administrative fee for each license verification will apply to
insurance companies, credentialing entities, and recruiting companies for
license verifications and letters of good standing. If you prefer to pay by
company credit card you must contact us by phone with your information.
Licensees requesting a letter of good standing to be sent out-of-state must
send a letter of request accompanied by a check for $25.00 made payable to the
WVBESLPA. You may also contact us by phone with the address information and pay
by credit or debit card.
Any affidavit requiring licensing information sent to any out of state
facility requires a letter of request accompanied by a $25.00 service fee.
